The Latino HealthCare Forum (LHCF) has undertaken a new agreement with Sendero Health in Austin, Texas to assist with providing value-added services, eligibility and enrollment for the Medical Access Program (“MAP”), IdealCare, STAR/CHIP and the Affordable Health Care Act through funding from the City of Austin. The LHCF will focus on the following four key campaign, to engag clients in a more meaningful way and thus captivate them to become more individually empowered to control their individual health the health of their families and their electronic health care records: Sendero projects: Healthy Baby, Healthy Mommy – Improving the well-being of mothers and infants Asthma Management – Asthma disproportionately affects poor and minority children. Empower families to self-manage their children Preventable Trauma – Safety for children; Help youth 12 – 19 and their families to engage in holistic approach to wellness Diabetes Prevention/Management EHR - Blue Button Connector
